First described in 1981, floppy eye syndrome (FES) is characterized by very elastic upper lids that become easily distorted and everted with minimal lateral traction. In a case-control study, Ezra et al. have found that FES appears to be strongly associated with obstructive sleep apnea-hypopnea syndrome (OSAHS) and keratoconus. The study involved 102 FES patients and a control group of patients from a diabetic retinopathy clinic who were matched 1:1 in terms of age, body mass index (BMI), and sex. Not only did they find significant associations between FES and OSAHS and keratoconus, but between lash ptosis, dermatochalasis, upper lid medial canthal laxity, upper lid distraction, palpebral aperture, and levator function as well. In addition, while FES was more common in obese males in their 6th decade, the condition affected patients with a significant range of ages, BMI, and sex. The authors urge clinicians to keep in mind this strong association between FES, OSAHS, and keratoconus when determining the course of treatment. \*From the April, 2011 issue of American Journal of Ophthalmology. Brown, beloved father, grandfather, husband, healthcare professional, and adventurer, who died on January 3, 2025, at the age of 86. He was the beloved son of Julian Delmore Brown (born in Fellows, CA) and Adrienne Paulette Henriette Gauzy (born in Aubervilliers, France), and brother of Diane Christner. He was predeceased by his wife, Carol Kidwell Brown, with whom he shared many joyful years. He is survived by his four children, Kim, Greg, Aimee, and Michelle, and seven grandchildren. Robert’s life was a remarkable journey of resilience, passion, and dedication to helping others. Born on February 19, 1938 in Oakland, California, Robert embraced the world with curiosity and ambition. After graduating from Berkeley High, he attended the University of California at Berkeley and earned his D.D.S. from UCSF. He started his Danville, California orthodontic practice in 1964. After selling that practice in 2010, he and his daughter, Aimee, built a TMJ and sleep apnea business together. Robert's dedication to providing genuine healing, rather than just treating symptoms, earned him the respect and admiration of all who knew him. His 60-year healthcare career concluded with his retirement in February 2024 at the age of 86. Beyond his work in orthodontics, Robert was a skilled chef who trained in France. In the early 1980’s, he and his wife, Carol, opened The Cuisinary, a cooking store and school in Danville. This became a gathering place for culinary enthusiasts and hosted legends including James Beard, Jacques Pépin, and Simone Beck. Robert’s love of food was only rivaled by his love of adventure. He pursued a wide variety of hobbies including building several homes at Donner Lake, tournament fishing in Mexico, abalone diving, gold dredging in the Trinity Alps, gardening, and water and snow skiing. He was a member of the Danville Rotary Club and was named a Rotarian of the Year. He founded Danville’s Discovery Counseling Center and the senior citizens Dial-A-Ride initiative. In 2014 Dr. Bob turned to writing, publishing his first book, "Why? The Question That Could Save Your Life," followed by "Punk Medicine." His writings reflected his belief in holistic health and longevity. His thirst for knowledge most recently led him to the study of quantum physics. In addition to his many professional accomplishments, Robert had a remarkable personal story. He survived being run over by a car at age seven, a broken neck from a football injury in college, heart attacks, cancer and two bouts of Guillain-Barré Syndrome related to Covid 19. He viewed each challenge as a temporary setback, and was always planning his next goal or adventure. After many years of cheating death, Robert finally succumbed to heart failure. Dr. Bob leaves behind a legacy of courage, compassion, curiosity and a thirst for life that inspired everyone around him. He will be deeply missed by his children, grandchildren, and countless friends, patients, and colleagues. Sleep disorders are extremely common in people of all ages. Sometimes it’s obvious to the patient that they suffer from a sleep disorder, but other times it may not be. If you experience any of the following, you may have a sleep disorder: You snore loudly – so loudly that your spouse may have to sleep in another room. You fall asleep easily at inappropriate times like movies or concerts. You wake up with sore teeth or a sore jaw (often caused by bruxism or teeth grinding). While sleep disorders can affect people of all ages, it’s particularly common in older patients. If you suffer from a sleep disorder, it can greatly impact the quality of your life. Dr. Alvarez specializes in treating sleep disorders and believes that resolving these issues can improve a patient’s overall health and well-being. One of the most serious of all sleep disorders is called sleep apnea. This condition can not only impact your sleep quality but can also be life-threatening. It occurs when a patient quits breathing for 10 seconds or longer while sleeping. Some symptoms of sleep apnea include: Dry mouth in the morning (caused by leaving the mouth open to breathe while sleeping). Lethargy, or being tired during the day (caused by lack of sleep at night). If you believe that you or someone in your family suffers from sleep apnea, we can help. We have been successfully treating this serious condition in our patients for years. We would be happy to discuss the symptoms and possible treatments with you to alleviate the problem. Poor concentration, difficulty performing work duties. Missing or extracted teeth (making less room for the tongue). Large tonsils, adenoids, and/or poor nasal airways. A sleep study is the next step. There are two types of diagnostic sleep tests, a PSG (Polysomnography) and HST (Home Sleep Test). Depending on your medical insurance, you may or may not need a referral from your primary care doctor for a sleep test. Also, depending on your medical insurance coverage, you may be eligible for an HST rather than a PSG. A PSG is more comprehensive than an HST in that it is a comprehensive recording of the biophysiological changes that occur during sleep. It is usually done at a sleep center. It monitors many functions, including the brain, eye movements, muscle activity, and heart rhythms. If the idea of sleeping overnight in a lab is keeping you from seeking a diagnosis, the HST may be a better option. An HST, also called an unattended sleep test, can be done in the comfort of your own bed, usually done over 1-3 nights. This test monitors oral and nasal airflow, oxygen levels in your blood, and respiratory effort. Even if your insurance does not cover an HST, there are currently companies that will provide you with an HST unit for under $300.00. In addition, if you just don’t want that diagnosis of “OSA” on your permanent medical record, paying out of pocket for an HST may be the answer. There are several different treatment methods for OSA. The treatment methods differ depending on the type of OSA you have and the severity of it. It is important that you know and understand all your treatment options, their pros and cons, and the risks vs. benefits of each option. It is also essential to know what you can do to try to decrease the severity of your OSA or possibly correct it. Changing sleeping positions to improve breathing and avoid sleeping on your back. Improving airways (removing large tonsils/adenoids, repair deviated septum, treat allergies, orthodontic arch development). Surgery. From soft tissue to jaw advancement, there are many options. The long-term effectiveness of several of them is unknown. Dental Devices, aka, Oral Appliance Therapy – WE DO THIS! OSA occurs when there are repeated occurrences of the upper airway being blocked during sleep. “Apnea” is defined as the cessation of breath. An oral appliance treats mild, moderate, and severe OSA by moving the lower jaw slightly forward. This movement tightens the soft tissue and the upper airway muscles, preventing obstructions while you sleep. The oral appliance also prevents the upper airway tissues from vibrating as air passes over them, thus frequently reducing snoring. Though oral appliances for treating sleep apnea are recommended primarily for patients with mild to moderate OSA, several patients are being effectively treated with severe OSA. We can know for sure by having a sleep test with the appliance in to confirm its efficacy. Then you can make a choice. There doesn’t have to be one answer. Some patients chose the oral appliance instead of the CPAP. Some patients alternate using the oral appliance for travel, camping, or when they need a break from the CPAP. In cases where the oral appliance reduced the apneas but not to 5 or less per hour, some patients chose a combination therapy where they used the oral appliance together with the CPAP, but the CPAP was set at a lesser, more tolerable pressure. Then there are those patients who can only tolerate the CPAP for a few hours before unconsciously removing it. Those patients can keep the oral appliance next to their bed and put it in upon removing their CPAP. In summary, you have choices and options. Take control of your treatment and do what is best for you. Oral appliance therapy is a medical service, not dental. Most medical insurances do cover this treatment method, including Kaiser and Medicare. Though we are out of network with many of the medical insurance companies, we can verify your benefits. We are in network with Kaiser Permanente. Outside of the insurance companies, we do offer no interest payment plans, Care Credit, and accept all major credit cards. The joints are flexible, allowing the jaw to move smoothly up and down and side to side and enabling you to talk, chew, and yawn. The muscles attached to and surrounding the jaw joints control the position and movement of the jaw. The TMJ are amazing joints, unique in the fact that they are the only joints in our body that not only rotate but slide, thus requiring the discs to slide with them, preventing bone-to-bone contact. When Temporomandibular Joint Dysfunction (TMD) occurs, one or both discs are not functioning properly, thus allowing bone-to-bone contact. This dysfunction can cause popping, pain, and discomfort, not to mention permanent damage to your joints and teeth over time. Malocclusion (misalignment of the way your teeth fit together). Holding the jaw in a prolonged unnatural position (musical instrument playing, dental procedure). Opening the mouth too widely (eating, yawning, singing). Improper habits (gum chewing, fingernail biting, moving the jaw laterally). Clicking, popping, or sandy sounds in your ear(s) when moving the jaw. Tooth pain (often leading to repeated root canals). Limited range of motion of the mouth or locking of the jaw. Difficulty chewing or tiredness of the facial muscles. Teeth clenching and/or grinding will exacerbate TMD. We have been treating patients with TMD since the early 1970s with TMJ Splint Therapy. We have relieved thousands of patients who were suffering. Our holistic approach, with the avoidance of the use of medications, is the key to our success. Rather than simply treating the symptoms, we strive to get to the root cause. A complete medical and dental history is taken on each patient. Depending on the patient’s history, appropriate referrals to specialists may be made in conjunction with TMJ splint therapy. Our office has an onsite lab, and we fabricate the splints ourselves, making for a perfectly balanced appliance. It depends on each individual policy. We are contracted with Kaiser Permanente, though you must seek a referral to us through your primary care physician. The referral department will then confirm if you have coverage for this type of treatment and refer you accordingly, usually to the Maxillofacial Department. Then that department refers you to us if needed. Most dental insurances, as well as some medical insurance companies, will cover TMD treatment. Though we are out of network, we will collect your insurance information, inform you of your benefits, and try to get you covered. This is described as a focal hyperirritability in muscle that can strongly modulate central nervous system functions. Travell and followers distinguish this from fibromyalgia, which is characterized by widespread pain and tenderness and is described as a central augmentation of nociception giving rise to deep tissue tenderness that includes muscles. Studies estimate that in 75–95 percent of cases, myofascial pain is a primary cause of regional pain. Myofascial pain is associated with muscle tenderness that arises from trigger points, focal points of tenderness, a few millimeters in diameter, found at multiple sites in a muscle and the fascia of muscle tissue. Biopsy tests found that trigger points were hyperirritable and electrically active muscle spindles in general muscle tissue. Trigger points have numerous qualities. They may be classified as potential, active/latent, key/satellites, and primary/secondary. There are few more than 620 potential trigger points possible in human muscles. When they become active or latent, these trigger points show up in the same places in muscles in every person. This means that trigger point maps that are accurate for everyone can be made. An active trigger point is one that actively refers pain either locally or in another location (most trigger points refer pain elsewhere in the body along nerve pathways). A latent trigger point is one that exists and does not yet refer pain actively but may do so when pressure or strain is applied to the myoskeletal structure containing the trigger point. Latent trigger points can influence muscle activation patterns, which can result in poorer muscle coordination and balance. Active and latent trigger points are also known as “Yipe” points, for obvious reasons. A key trigger point is one that has a pain referral pattern along a nerve pathway that activates or creates a latent trigger point on the pathway. A satellite trigger point is one that is activated by a key trigger point. Successfully treating the key trigger point will often resolve the satellite, either converting it from being active to latent or completely treating it. In contrast, a primary trigger point in many cases will biomechanically activate a secondary trigger point in another structure. Treating the primary trigger point does not treat the secondary trigger point. Activation of trigger points may be caused by several factors, including acute or chronic muscle overload, activation by other trigger points (key/satellite, primary/secondary), disease, psychological distress (via systemic inflammation), homeostatic imbalances, direct trauma to the region, accident trauma (such as a car accident which stresses many muscles and causes instant trigger points) radiculopathy, infections, and health issues such as smoking. Trigger points form only in muscles. They form as a local contraction in a small number of muscle fibers in a larger muscle or muscle bundle. These, in turn, can pull on tendons and ligaments associated with the muscle and can cause pain deep within a joint where there are no muscles. When muscle fibers contract, they use biochemical energy. Depleting these biochemicals leads to the accumulation of fatigue toxins such as lactic acid. The tightened muscle fibers constrict capillaries and prevent them from carrying off the fatigue toxins to the body’s recycling system (liver and kidneys). The buildup of these toxins in a muscle bundle or muscle feels like a tight muscle—a slippery elongate bundle. When trigger points are present in muscles, there is often pain and weakness in the associated structures. These pain patterns in muscles follow specific nerve pathways and have been readily mapped to allow for the identification of the causative pain factor. Many trigger points have pain patterns that overlap, and some create reciprocal cyclic relationships that need to be treated extensively to remove them.
